Health Home Program
The Health Home Program is here to help families navigate the complicated health care, social services, and educational systems. A Health Home is not a physical location or home, but rather a team of people to assist you in working with your doctors, school administrators, and other providers. We also help you find and enroll in services you may need. We make sure everyone is communicating and providing you the best service.
Many people with multiple chronic and serious conditions struggle to find the right combination of services. As a Health Home client, you will have a dedicated Resource Coordinator who will be your guide and partner, teaching you how to connect to the right services and providers. The Resource Coordinator works with you to understand the steps needed to care for yourself or your child and ensure everyone involved in your care is working together. With these skills, you’ll gain confidence in managing your or your child’s care both now and in the future.

Program Details
Health Homes Program - Care Coordination
You will be assigned a Resource Coordinator who will help you identify and access needed services within your community. The Resource Coordinator will connect and collaborate with all services providers to ensure all your needs are met. In addition to the Resource Coordinator, your or your child’s Health Home team may include:

Mental health and/or substance use providers

Health care providers
-
School personnel
-
Community members and/or other family members who you feel can assist
Visits with members of your Health Home team take place in the community: at home, school, a clinician’s office, or wherever you feel most comfortable.
To be considered for the Home Health Program, clients need to demonstrate all of the following:

Enrolled in Medicaid

Living with two or more chronic conditions or one single qualifying condition. Single qualifying conditions include:

serious emotional disturbance (under age 21)
-
serious mental illness (ages 21+)
-
HIV/AIDS
-
sickle cell disease
-
complex trauma
-
All ages accepted, including adults

Hours: Office — Monday-Friday, 9 a.m. - 5 p.m.; Staff available 24/7 to enrolled families via cell phone

Services provided: Care coordination

Admission criteria: Services are for families with a high-needs child or adults who have at least two chronic conditions or SED (severe emotional disturbance) and must be enrolled in Medicaid. Referrals may be made directly.
