Home Visiting

At The Child Center of NY, we believe that all children — even those experiencing the most serious problems — deserve the chance to get the help they need while remaining with their families. For children at risk of psychiatric hospitalization, we offer 24/7 home-based crisis intervention, case management, and coping skills training through programs of varying intensity.

Program Details

Children and Family Treatment and Support Services (CFTSS) Program
  • Hours: Monday-Sunday, 9 a.m. - 9 p.m. (Additional hours by appointment)

  • Services provided: CFTSS provides clients facing some of the toughest mental health challenges with in-home therapeutic services, psychiatric support and treatment, and skill-building services, as well as aid with self-advocacy, and family and youth peer support services.

  • Admission criteria: Clients must have Medicaid Managed Care, and a completed referral, indicating the reason for referral. We serve clients in Queens, Brooklyn, Bronx, and Manhattan, and in Nassau, and Suffolk counties.

Home-Based Crisis Intervention (HBCI)
  • Hours: Office — Monday-Friday, 9 a.m. - 5 p.m.; staff available 24/7 to enrolled families via cell phone

  • Services provided: Intensive home-based interventions for families of youngsters who are already involved with the mental health system. These are short-term services to stabilize youngsters who are at imminent risk of psychiatric hospitalization. Services are offered in the home and referrals are made for follow-up services when intervention is completed.

  • Admission criteria: Youngsters must have used mental health services in the past. Referrals may be made directly. Medicaid and many third-party insurance plans are accepted. We also serve undocumented clients and clients not eligible for Medicaid.

  • Languages spoken: English and Spanish

  • Ages accepted: 5 to 20

Home and Community Based Services (HCBS)
  • Hours:  Office — Monday-Friday, 9 a.m. - 5 p.m.; staff available 24/7 to enrolled families via cell phone

  • Services provided: HCBS provides an array of developmentally and culturally appropriate services to children and their families to prevent the onset and/or the progression of any behavioral health conditions. Through caregiving support; resources building, sharing, and advocacy; pre-vocational training through job and education preparedness; and short-term planned respite breaks, the goal is to identify the need within the client early to prevent any further restrictive services and/or placements while allowing for the stabilization of the child to occur within their current living environment.

  • Admission Criteria: Young people and their families can access HCBS services through their Health Home Care Manager, who would make the referral to the program. As the program is funded through State and Federal Medicaid, those children who have private health insurance are eligible but would have to go through a separate eligibility process.

Step-Down Program

  • Services provided: Intensive home-based interventions for families of youngsters who are already involved with the mental health system. Youngsters must be experiencing serious emotional and/or behavioral difficulties.

  • Admission criteria: Youngster must be a resident of Queens County and must have used mental health services in the past. Referrals may be made directly. Medicaid and many third-party insurance plans are accepted. We also serve undocumented clients and clients not eligible for Medicaid.

  • Languages spoken: English, Spanish

  • Ages accepted: 2 to 18

  • Transportation: Subway—E or F to Union Turnpike–Kew Gardens station; Buses—QM1, QM5, QM6, QM7, QM8, QM18, QM21, X63, X64, X68.
